Consent and Authorised Representatives
Capture general and procedure consent without replacing the patient's identity or losing representative authority details.
Before starting consultation
- Open the patient's queue entry and check the Consent column. If general consent is missing, select Capture consent.
- Confirm the patient name and patient number shown in the consent window before anyone signs.
- Read or explain the displayed form to the patient or authorised representative and answer questions before continuing.
Tip: Do not use another person's patient record to save time. Consent evidence is permanently linked to the selected patient and clinic.
When the patient signs
- Leave the representative option off. The signer name should remain the patient's name.
- Ask the patient to sign in the patient signature area.
- Confirm the selected practitioner is the nurse responsible for the consent and that their saved practitioner signature is displayed.
- Save the form and wait for the Consent saved confirmation before closing it.
When a representative signs
- Turn on the authorised representative option. Enter the representative's full name—the patient name must remain unchanged elsewhere on the form.
- Record the relationship to the patient, for example parent, legal guardian or mandated person.
- Record the authority relied on, for example parental responsibility, guardianship order or written mandate. Do not place the authority in the relationship field.
- Capture the representative's signature and the selected practitioner's signature, then save.
Tip: For minors or a person unable to consent, follow the clinic's approved capacity, guardian and assent procedure. The software records evidence but does not decide legal authority.
Additional procedure consent
- In Consent for today, confirm General consent active and tick the routine consultation confirmation.
- Select only the additional procedure actually being performed.
- Complete and save the procedure-specific form. A selected procedure must have a signed form before the consultation can be completed.
- If the nurse-signature warning appears, stop and have that nurse save their practitioner signature in their own profile before retrying.
Verify the record
- Open the completed consultation and find Consent record.
- Open each saved form and confirm patient, signer capacity, signer name, relationship, authority, signatures, template version and signed date.
- If the system says evidence could not be displayed, use Retry consent records. Do not assume this means consent is missing.
